The National Academy for State Health Policy convened a national forum of experts representing state and federal health agencies, families of CYSHCN, providers, and health plans to discuss strategies and opportunities to improve high-quality, equitable care coordination for CYSHCN. This event built upon NASHP’s work developing and supporting the implementation of the National Care Coordination Standards for Children and Youth with Special Health Care Needs. The forum centered on two key topics for improving care coordination systems for CYSHCN: integrated care and the care coordination workforce. The proceedings capture opportunities, strategies, and innovations to improve care coordination shared by forum participants.
