Purpose
Self-Management Supports (SMS) are vital and cost-effective measures that help families manage their child’s complex conditions at home. While it is known that self-management supports increase quality, improve health outcomes, and reduce health inequities, they are underutilized in pediatric care. Building on a successful pilot study, the AFFIRM California statewide survey will help us understand the degree to which families of children and youth with special health care needs (CYSHCN) are receiving SMS from their health care providers. This work will produce a reliable survey that can be used to identify gaps and inequities in the provision of SMS to families. Results will help policymakers and health care providers deliver tailored support to families with the greatest needs.
Grant Amendment – September 2024
This grant amendment will enable the AFFIRM project team to expand the study beyond its original target of eight clinics and hospitals across the state to a total of 16 confirmed sites. The additional sites will strengthen the study by providing a more complete picture of the extensive diversity and experiences of California’s families. The sites represent urban, suburban, and rural areas, border regions, under- and well-resourced communities, and a wide variety of population groups with racial and ethnic diversity.
