Purpose
To develop a care coordination landscape assessment report to inform policy development in health and social services for children with complex needs.
Outcome
Care coordination often involves services from multiple public programs administered by different agencies. The extent to which those agencies have a shared concept of care coordination and implement that concept uniformly will affect the recipients’ experience of care integration. The project was intended to collect information from California state departments serving children and families about their definitions of care coordination and how they and their affiliates were providing care coordination services. The project was intended as a first step toward harmonizing care coordination policy among California state departments. For a variety of reasons the project was not completed, and unused funds were returned.
