At its first roundtable in a series focused on young adults with chronic conditions, Generation Patient convened a multi-stakeholder learning community to identify challenges that adolescents and young adults face during the transition from pediatric to adult health care. Co-authored by young adults with chronic conditions and published in Health Care Transitions, the roundtable proceedings outline key barriers to transition of care, including the need for improved self-advocacy skills, tailored care approaches for patients with complex care needs, and systemic supports such as transition navigators and social workers.
The authors recommend enhancing patient independence during pediatric care, adopting a multidisciplinary team approach, and implementing long-term system reforms such as improved adult provider training and Medicaid policy changes as solutions for a smoother health care transition.
