For the evaluation of a pilot project offering intensive care management and coordination to children and youth whose social circumstances contribute to poor medical management of their chronic condition

Patients’ personal and social circumstances affect their health and health behaviors. For children with chronic health problems, those factors can interfere with the management of their conditions, possibly impairing their health and functioning, and even jeopardizing their lives. The Novel Interventions in Children’s Health (NICH) model of intensive, long-term case management was developed at the Oregon Health & Science University to address these situations. This project was designed to replicate the NICH program at the Lucile Packard Children’s Hospital and evaluate its impact on health care utilization and other patient outcomes. The replication was successful, and funding from the Helmsley Charitable Trust was obtained to continue the program.
