Purpose
Self-Management Supports (SMS) are vital and cost-effective measures that help families manage their child’s complex conditions at home. While it is known that self-management supports increase quality, improve health outcomes, and reduce health inequities, they are underutilized in pediatric care. Building on a successful pilot study, the AFFIRM California statewide survey will help us understand the degree to which families of children and youth with special health care needs (CYSHCN) are receiving SMS from their health care providers. This work will produce a reliable survey that can be used to identify gaps and inequities in the provision of SMS to families. Results will help policymakers and health care providers deliver tailored support to families with the greatest needs.
