Purpose
Generation Patient aims to improve the quality of life for young adults with chronic medical conditions who are transitioning from pediatric to adult health care. This small grant supports Generation Patient’s planning process to create a Roundtable on Young Adults with Chronic Medical Conditions. The Roundtable will consist of a multi-stakeholder learning community that will engage in a series of virtual discussions to address young adult patients’ most pressing issues as they transition out of the pediatric health care system.
The planning process includes establishing both a Patient Advisory Committee and a Medical and Research Advisory Committee. The two Advisory Committees will discuss what systems-level change means to them, what systemic challenges patients face, and which key stakeholders can inform on these issues. The project team will conduct a brief literature review on the identified topics and select four to six systems-level issues for the Roundtable to address.
