Purpose
To identify current policies and programs, best practices, and state policy options for improving care coordination for CSHCN in California.
Outcome
This project was intended to provide guidance to those in California interested in and/or responsible for providing care coordination to children with special health care needs and their families. The project reviewed exemplary care coordination programs in six other states and described how they dealt with issues of eligibility determination, service standards, reimbursement, financing and oversight. The experience of these states was analyzed and interpreted in the context of California’s various programs serving these children. Options for addressing each of these issue areas in California were provided, and guidance was also offered as to what the state would need to do to provide an effective, sustainable care coordination system.
