Improving Pediatric to Adult Health Care Transitions in Medicaid: MACPAC’s Recommendations to Congress

In a June 2025 report to Congress, the Medicaid and Children’s Health Insurance Program Payment and Access Commission (MACPAC) offered recommendations for improving transition from pediatric to adult care for children and youth with special health care needs (CYSHCN) covered by Medicaid. This report was informed by the work of the National Alliance to Advance Adolescent Health, a partner of the Lucile Packard Foundation for Children’s Health. As Congress has since passed legislation that will significantly cut funding for Medicaid and reduce millions of Americans’ access to health care, consideration of transitions in the health care system is as important as ever.
