The National Alliance to Advance Adolescent Health’s Got Transition program, in partnership with the American Academy of Pediatrics, released the 2025 Coding and Payment Resource to support the delivery of and payment for recommended transition services in pediatric and adult primary, specialty, and behavioral health care settings. 

The resource, intended for health care professionals (e.g., administrators, providers, billing specialists), provides guidance on current procedural terminology (CPT) codes for transition-related services and payment, clinical vignettes on health care transition, and an insurance carrier letter template that can be used and adapted to encourage payer recognition of transition-related codes. The resource includes the current Medicaid fees and relative value units (RVUs)—a standardized measure used to determine the value of a medical service—for transition-related codes.
