CARE COORDINATION

California Community Care Coordination Collaborative

For more than five years, the California Community Care Coordination Collaborative (5Cs) improved local systems of care for children and youth with special health care needs. Coalitions were organized by county and brought together staff from the California Children’s Services Program, regional centers, family resource centers, and Early Start, as well as pediatricians, public health nurses, and special education and mental health professionals. Effective care coordination requires strong interagency communication. Through convenings, the 5Cs coalitions had a structured opportunity to learn from one another, identify areas of shared need, discuss emerging challenges, and connect with others engaged in improving quality of services.

We are not currently funding 5Cs coalitions. If you’re interested in starting a 5Cs coalition in your county, please contact us.
